Recent Reform of the German Long-Term Care Insurance - Implication of PGB Pilot Project for Korea -

Abstract
This study analyzes the background and consequences of the recent reform of German long-term care(LTC) insurance so as to find the implication for Korean LTC insurance. In Germany, PGB(personal budget) is given to beneficiaries instead of LTC benefits in kinds, so that they can select services freely according to their needs. This reform is to enhance satisfaction of the beneficiaries with residual services of LTC insurance and to boost usage. Reform of LTC insurance in Germany was inevitable because medical approach to the LTC could not adapt individual living condition and needs of beneficiaries. Introduction of PGB brought new challenges to the beneficiaries. The beneficiaries had to search for appropriate services and reasonable fees and quality of services should be evaluated by themselves. To solve these problems, case management is crucial and social workers played a important role in managing cases. This study shows that social welfare approach seems to become more popular in the German LTC insurance. By the way, in Korea only serious LTC cases are to be entitled to use benefits, so that medical approach and institutional care are likely to become dominant. This situation is very similar to German LTC insurance before recent reform. But coverage of the Korean LTC insurance should extend to residual care in oder to reduce expenditure and to gain more acceptance of the enrollee. In that case, it would very useful to lean from experience of recent reform of LTC insurance in Germany.
